Instructions for Applications
- Download, read, and print a copy of Allied Health Checklist, and keep for your records.
- Download and complete the Allied Health Professional application and Sponsorship of Allied Health Professional forms.
- No electronic or fax copies are being accepted at this time. Please mail all original, completed, and signed forms.
If you are a Non-QMC Nurse Researcher, you will still need the documentation listed here, however please coordinate all your application documentation, approvals, and authority implementation with: Renee W. Latimer, BSN, MPH
